Methodology and sources
Coverage
Assisted Living Inspections covers Texas today, with more states planned. Each state's pages are built only from that state's own public records, and they live under the state's name (Texas is at /texas/). Everything below describes the Texas data.
Texas sources
- HHSC assisted living directory (apps.hhs.texas.gov, AL.xlsx). This is the list of licensed facilities: license number, type, address, licensed capacity, Alzheimer's certification and certified beds, and owner. The roster used here is as of 10/28/2026.
- TULIP, the HHSC long-term care provider search (tulip.hhs.texas.gov). This is where the inspection citations (visit exit date, visit type and correction date) and enforcement actions (action, date, amount and rules cited) come from.
We read only public pages, without logging in, one facility at a time with a pause of several seconds between requests, and we follow each site's robots.txt.
What we show, and what we leave out
We show facts: dates, visit types, counts, correction dates, penalty amounts and bare rule numbers. We don't store or display the state's narrative text for citations or penalties. To read it, use the TULIP link on each facility page.
We also leave out personal information that appears in state files, including administrator names, email addresses, and the names and mailing addresses of owners who are individuals rather than companies.
Freshness
HHSC republishes the directory regularly, and TULIP updates as inspections close. We plan to refresh at least monthly. Each facility page shows the date its record was collected. This build combines: statewide TULIP collection run (2,012 facilities statewide, built Oct 30, 2026).
Metro areas
Metro pages group counties using the federal (OMB) metropolitan statistical area definitions: Houston, Dallas–Fort Worth, San Antonio, Austin and El Paso. Every other county is listed under Texas. A city whose facilities span two counties is listed under the county where most of them are. Where the state roster has an obvious spelling variant of a city name, or a missing county, we normalize it (for example, the county is taken from other facilities in the same ZIP code).
No ratings
We don't score or rank facilities. Lists are sorted alphabetically or by an objective field you can see, such as certified beds.
Featured placements
Some pages have space for a clearly labeled "Featured" placement that a facility or service could pay for. Placements never change a facility's data, its position in lists or search, or whether its citations are shown. There are no featured placements on the site yet.
Corrections
If something here doesn't match the state record, the state record wins. Reports of errors will be handled through a corrections contact once the site launches.
